Permits

First Time Applicants (excluding Firearms) 

• Current Valid State-issued Driver’s License
• Current Valid State-issued ID Card

  AND one of the following:

• U.S. Passport
• U.S. Military ID Card
• U.S. Birth Certificate
• Certificate of Naturalization
• Resident / Employment Authorization Card

 Renewal ONLY - you will need one of the following:

• Current Valid State-issued Driver’s License
• Current Valid State-issued ID Card

Regulated business permits for:

• Alcohol Sales Establishments:
•   (restaurants, nightclubs, package stores, etc.)
• Adult Entertainment Establishments:
•       a) entertainers/dancers
•       b) alcohol servers/bartenders
•       c) all other employees
• Carnival operators
• Escort/dating
• Pawn Shops
• Precious metal dealers/employees
• Door to door sales

Private parties where alcohol is being sold require a permit. 

Current valid State-issued driver’s License, current valid state-issued ID, or US Passport

• U.S. citizens born abroad and applicants who are naturalized U.S. citizens must provide a U.S. passport, certificate of naturalization, or resident alien card, in addition to their state-issued driver’s license or state-issued ID
• Probate Court Application and $5.00 cash.
